Course Content
This training session will explore ways advance care planning conversations and information can be recorded and shared with all involved in a patients care across different services.
Eligibility Criteria
Registered Nurses (including Associate Nurses) or Allied Healthcare Professionals working within adult wards in HDFT or in community setting (note: limited places may be available for medical staff, if free places).
