St Gemma’s Out-Patients, based at the Hospice, can give you extra support to manage your symptoms, gain more confidence at home and help maximise your quality of life at any stage of your illness.

Supporting patients and their carers

We offer a wide range of Out-Patient services to provide you with information and support that enables you make the most of your quality of life and self manage any symptoms you may be experiencing.

You can come along and enjoy the friendly and therapeutic environment; you can access creative therapy activities, massage and aromatherapy, grow plants in our glass house, and enjoy our beautiful gardens.

Our sessions can be booked without a referral, or you may be referred to one of our specialist information or assessment services.

Out-Patient Sessions

Fitness

Seated Exercise

A Physiotherapy led chair based exercise class for all levels and abilities. The class is fun, friendly and the exercises are easily adaptable to you. Each session lasts 1 hour, this includes: introductions, warm up, exercises, cool down and discussion.

Balance Group

A physiotherapy led class to help your balance, co-ordination and confidence to exercise.

Fitness games

A fun, friendly and relaxed multisport games session, aimed at all levels and ability

Movement to Music

Seated Zumba class  – Fun and exercise to music

Breeze

Are you experiencing symptoms of breathlessness, anxiety or fatigue? Join our virtual 6-week programme BREEZE (by referral)

Breathlessness, anxiety and fatigue are common problems for those living with a life-limiting illness. This virtual 6-week programme called the Breeze programme, aims to help equip you, whether you’re a patient, family member or carer, with the tools and understanding to manage these difficult symptoms.

We aim to improve the quality of your life, help you develop strategies, increase your confidence and help you put control back into your life. Each session will include self -management advice and wellbeing resources.

The Breeze Programme

Session 1: Understanding breathlessness, breathing strategies, techniques and positioning, general management tips.
Session 2: Energy conservation and lifestyle management
Session 3: Anxiety management; understanding stress, anxiety and panic
Session 4: Relaxation and having a good night’s sleep
Session 5: Nutrition and managing a poor appetite; benefits of exercise
Session 6: Staying Active – how to utilise the session information to manage your symptoms.

Accessing the Service

To take part in the Breeze Group you can either ask your GP, your St Gemma’s Community Nurse Specialist (CNS), or if you are attending Out-Patients you can ask any of the nursing staff to refer you or contact the Out-Patients team. After we have received your referral we will contact you to arrange an assessment for the group.

After this we can arrange a start date with you; we can establish which session would be best for you to start depending on your priorities and symptoms.

Carer’s Café

St Gemma’s Carers’ Café provides support for those caring for someone with a life-limiting illness. The group offers a safe space to relax, meet others and seek advice.

I love my time in the art room at St Gemma’s Out-Patients. Patients dip in and out as much as they wish and I like that. It’s very social, chatting to other people about a common interest and finding out more about them. The staff help as much as you need or leave you to it, if that’s what you’d prefer. They know when you may need support or when to step back.

Art is now my main hobby, form of pain relief and therapy. I look forward to my visits so much and take a real pride in making things and bringing them home.

The service offers support to patients and their carers in a safe and welcoming environment, through a wide range of services such as out-patient clinics and group sessions.

We offer a number of different Out-Patient services, which are aimed at providing you with information and support you require to enable you to self-manage and make the most of your quality of life. Services are available for both patients and their carers. You can book onto one of our group sessions without a referral.

You can come along and enjoy the friendly and therapeutic environment; you can access fitness, creative or wellbeing therapy activities. Patients can attend an outpatient appointment with our Physiotherapists, Occupational Therapists, and Complementary therapists, Nurses or Doctors. Carer support groups and bereavement services are also available.
