You may be referred to St Gemma’s Day Services to attend a period of assessment and support. Patients who attend on these Tuesday sessions attend regularly for up to 12 weeks, depending on their needs.
Prior to attending we will arrange to be in contact with you to establish your needs and determine what we would be able to offer to support you. Patients can either travel independently to the weekly sessions, or we have some volunteer drivers or ambulance transport that can be arranged.
We aim to help you with a variety of issues:
- managing symptoms such as pain, sickness, fatigue
- providing emotional support to help you adapt to difficult situation, address anxiety or other concerns
- offering advice about practical and financial issues
- offering information and support in planning for the future
- offer support to enable you to self-manage your symptoms
Many people value the support of meeting with others in a similar situation. Your family or carer can have a break knowing that you are safe. Patients arrive about 10am and stay with us till about 3pm, depending on transport. We will offer you lunch during your visit and we offer a variety of activities during the day including art and crafts, exercise and relaxation sessions.You could have one to one sessions with a member of our therapy team, the nursing staff or the doctors, depending on what we have established your needs of the service are.
It may be also be arranged for you to visit the Hospice as an Out-Patient to see a nurse, doctor, advanced nurse practitioner or therapist.
Our team liaise with the other professionals that are involved in your care, aiming to give you a continuous service.
