St Gemma’s Out-Patients

St Gemma’s Out-Patients, based at the Hospice, can give you extra support to manage your symptoms, gain more confidence at home and help maximise your quality of life at any stage of your illness.

The service offers support to patients and their carers in a safe and welcoming environment, through a wide range of services such as out-patient clinics, drop-in sessions and a number of symptom-management groups such as breathlessness and fatigue.

We offer a number of different Out-Patient services, which are aimed at providing you with information and support you require to enable you to self-manage and make the most of your quality of life. Services are available for both patients and their carers. You may be referred to one of our information or assessment services, or you can book onto one of our sessions without a referral.

You can come along and enjoy the friendly and therapeutic environment; you can access creative therapy activities, massage and aromatherapy, grow plants in our glass house and enjoy our beautiful garden.

Our expert staff include specialist nurses, doctors, social workers and therapists who work closely as a multi-disciplinary team.

Out-Patients Activities

We run a wide range of activities designed to support patients and their family members and carers. Please see below for examples of the regular activities we run.

Exercise Session

 

Session Type Session Name Description
Fitness Seated Exercise A Physiotherapy led chair based exercise class for all levels and abilities. The class is fun, friendly and the exercises are easily adaptable to you. Each session lasts 1 hour, this includes: introductions, warm up, exercises, cool down and discussion.
Balance Group A physiotherapy led class to help your balance, co-ordination and confidence to exercise.
Fitness games A fun, friendly and relaxed multisport games session, aimed at all levels and ability
Movement to Music Seated Zumba class  – Fun and exercise to music

 

Connection Stop and Chatter Come and join us for a cuppa. Opportunity to come and speak to staff and other patients. For 2 of the sessions in the week we have a PAT dog visiting

 

Bereavement café

 

This is a space for people who have had similar experiences to meet and support each other in a relaxed atmosphere. Those attending the group will have a link with St Gemma’s with their family member or friend having been cared for by one of our services in the last 2 years.
Carer’s Cafe An opportunity for carers to meet and share experiences. Group by referral only and facilitated by the Family Support Team.
Quiz A fun generalised quiz to test your brain cells, run by are excellent volunteer quiz masters Janet and David. Enjoy some tea/coffee and cake during the quiz.
Wellbeing Mindfulness Mindfulness is a self-help skill that can change the way we relate to our everyday experiences by learning to be more present.  You will be guided through different meditations that you can practice at home and incorporate into your daily life and routine.
Tai Chi Tai Chi is a sequence of slow flowing, continuous movements which relax, strengthen and energise the body while allowing the mind to rest, thus bringing about physical and mental wellbeing. Exercise will mainly be seated but can be advanced to standing for those that are comfortable.
Creative Arts and crafts Guided art or craft session, all materials supplied

 

Men’s Shed We will be offering sessions covering woodworking / craft skills suitable for patients to use/improve the skills /capabilities to produce products they will enjoy creating.
Gardening Group Practical gardening activity, inside if poor weather or in our beautiful gardens if the weather is fair. Activities will involve light exercise only and should be a manageable for all. The team will be on hand to provide expertise and assistance as required.

Unfortunately we are unable to provide ambulance patient transport. Please bring with you any medication you might need during your stay. We look forward to welcoming you.

Breeze

Are you experiencing symptoms of breathlessness, anxiety or fatigue? Join our virtual 6-week programme BREEZE (by referral)

Breathlessness, anxiety and fatigue are common problems for those living with a life-limiting illness. This virtual 6-week programme called the Breeze programme, aims to help equip you, whether you’re a patient, family member or carer, with the tools and understanding to manage these difficult symptoms.

We aim to improve the quality of your life, help you develop strategies, increase your confidence and help you put control back into your life. Each session will include self -management advice and wellbeing resources.

 

Session 1: Understanding breathlessness, breathing strategies, techniques and positioning, general management tips.
Session 2: Energy conservation and lifestyle management
Session 3: Anxiety management; understanding stress, anxiety and panic
Session 4: Relaxation and having a good night’s sleep
Session 5: Nutrition and managing a poor appetite; benefits of exercise
Session 6: Staying Active – how to utilise the session information to manage your symptoms

 

Accessing the service
To take part in the Breeze Group you can either ask your GP, your St Gemma’s Community Nurse Specialist (CNS), or if you are attending Out-Patients you can ask any of the nursing staff to refer you or contact the Out-Patients team. After we have received your referral we will contact you to arrange an assessment for the group.

After this we can arrange a start date with you; we can establish which session would be best for you to start depending on your priorities and symptoms.

Carers’ Cafe
